Summary
Under the direction of the Homeless Drop In Center, the outreach worker will provide Outreach services to the homeless population in the designated areas, provide referral and linkage back to the Drop in Center.

 
 

Essential Functions

TOp
  Establish contact with homeless people on the streets in South Los Angeles.
Build rapport with individuals
Conduct a short intake with clients
Inform and educate homeless individuals of services available to them at the Drop in Center
Provide referrals to appropriate agencies to address client needs.
Engage clients on various occasions to ensure that they are aware of services .
Input client information into HMIS
Maintain accurate log of client contact in the field
Outreach and develop working relationships with other agencies whose services will be beneficial to clients.
 
  Secondary Functions

  Attend all meetings and activities as they relate to this grant.  
 

Minimum Qualifications - Knowledge, Skills and Abilities Required

Knowledge:

TOp
  Experience as a homeless person and/or a minimum of 2 years of conducting street outreach or working with the homeless population  
  Skills:
TOp
  Bilingual Spanish preferred
Basic computer word processing skills necessary
Ability to speak and write effectively
 
  Abilities:
TOp
  Ability to work with and communicate effectively with the homeless and minority populations
Ability to work non-traditional schedule on occasion
 
  Supervisory Responsibilities

  The position does not have any supervisory responsibilities  
  Environmental Conditions (Working Conditions)
TOp
  This job includes walking outdoors and entails engaging homeless persons on the streets. Incumbent may be exposed to verbally and/or physically aggressive clients.  
  Physical Requirements

  This position typically requires at least 20 hours of walking /20 sitting weekly and carrying (max. 10lbs) of client outreach materials  
  Mental Requirements
TOp
  Candidate must be able to follow oral and written assignments, work with clients from diverse cultural backgrounds and ethnicities and engage homeless individuals and families on a regular basis.

  Summary

  Under the supervision of the Homeless Services Director, the After Care Specialist will have the overall responsibility for the following job functions:  
  Essential Functions
TOp
 
  1. Assist with community outreach and client recruitment for the HITOP Project
  2. Participate in orientations, intakes, and assessments to clients who are referred for homeless  services including HWCP, HITOP and The Drop in Center
  3. Orient client to services that are provided through HOP ICS
  4. Conduct housing, benefits, employment and other post treatment/program needs assessments
  5. Assess client eligibility for programs within HOP ICS and other community based organizations and assist with placement into appropriate programs
  6. Provide information to the client regarding post treatment housing options and assist with identifying, applying for and transitioning into independent living.
  7. Assist clients with identifying employment training, gainful employment and or education opportunities
  8. Develop an individualized post services care plan to assist clients with a successful transition to independent living.
  9. Follow-up with clients on a weekly basis post treatment to ensure stability
  10. Participate in development, facilitation and coordination of post treatment curriculum
  11. Maintain interagency consultation, coordination, and referrals.
  12. Help Process and facilitate client’s case with applicable agencies and departments.
  13. Maintain up-to-date and accurate documentation in client files
 
  Secondary Functions

  1. Attend meetings associated with HITOP and HWCP and other projects as assigned by Supervisor.
  2. Maintain files/records on client services in compliance with HIPAA and other funding requirements for auditing purposes..
  3. Perform other duties as assigned by the Homeless Services Director.
 
 

Minimum Qualifications – Knowledge, Skills and Abilities Required

  1. Bachelors Degree in Social Services or other Behavioral Health field is required from an accredited or state approved college or university, or Certified Alcohol and Drug Abuse Counselor with a minimum three (3) years job related experience.
  2. If in recovery, a minimum of three (3) years of being drug and alcohol free is MANDATORY.
  3. Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
  4. Bilingual preferred, but not necessary.
  5. Ability to communicate effectively, both written and orally.
  6. Proof of employment eligibility is required.
 
  Non-Essential Qualifications (optional) – Knowledge, Skills and Abilities

  1. Working knowledge of Microsoft Word, Excel, and other database programs is desirable.
  2. Knowledge of various community based treatment providers is desirable.
