| Community Assessment Service Center (SPA 6) MINUTES - July 07 | ||

5715 S. Broadway, Los Angeles , CA 90037 . The Community Assessment Service Center (CASC) provides services to persons deemed to be eligible for services. Participants are General Relief (GR) and CALWORKs clients, walk-ins and individuals who have been referred to us by the court (Proposition 36) for assessment and placement into treatment. The CASC utilizes a computerized substance abuse and mental health assessment, intake and referral system. There are referrals for both residential and nonresidential treatment services and other ancillary services such as emergency shelter, transitional housing, domestic violence and HIV/AIDS resources. We also have a collaborative agreement for services which includes a licensed R.N., two (2) probation officers and a mental health specialist on site as well as collaborative agreements for offsite services with Kedren, S.H.A.R.P. and Shields for Family. CASC staff completes a clinical assessment for each participant using the ASI to determine the appropriate level of treatment intensity, based on the participant's severity of addiction and the assessor's professional judgment. The assessor contacts a facility to see if there is a slot available. If so, then an appointment is made for admission for treatment. Client information is entered into our tracking and referral system within one (1) working day of the assessment. | HOP/SSG Outreach Program | ||
HOP's outreach program targets the dually diagnosed, parolees, substance abusing individuals and subpopulations of all individuals through peer-based street outreach, agency referral, LAHSA referrals or walk-in self referrals. Outreach links homeless individuals with formal helping agencies and/or other community based services. All of HOP's outreach staff are formerly homeless. Individuals are referred to our downtown office at 333 South Central Ave. , or to our office at 5715 South Broadway . They may also be referred for special housing needs assessment, emergency services and case management. Substance abusers are referred to the Community Assessment Service Center (CASC) for drug treatment and placement. Housing is the priority for all clients contacted in outreach and enrolled in services. Clients do not have to pay for outreach services or referrals. All homeless clients are served regardless of ability to pay. We have a bilingual staff for participants that only speak Spanish. The staff located at 333 S. Central are Raymond Blackshire, Junior Mitchell, Eloise Williams, Rinnah Hunnicutt, and James Jackson. The staff for the outreach program located at 5715 S. | HOP/SSG South Central Drop-In Center | ||
5715 S. Broadway, Los Angeles , CA 90037 . The South Central Drop-In Center 's purpose is to provide supportive services to homeless individuals with co-occurring disorders, to assist in obtaining housing, benefits, and to improve treatment outcomes. The center will increase skills, increase income, stability and self-sufficiency. ICS also provides supportive services such as case management, drug and alcohol treatment services and mental health treatment services. Individuals that come to the drop-in center are given food and clothing. An array of linkages to services is also provided for clients of the drop–in center. Participants also attend group sessions to prepare them for treatment. The bilingual staff includes Iris Leary, Saundra Minor, Denise Logan, Alex Chavez and Patrice Clausell. |

| ICS/SSG YOUTH Program | ||
5715 S. Broadway, Los Angeles , CA 90037 . The Youth program is an outpatient Alcohol and Drug treatment program for participant's ages 12-17 years old. The program uses the evidence based model Cannabis Youth Treatment (CYT) which includes the Motivational Enhancement Therapy/Cognitive Behavior Therapy (MET/CBT) approach with our participants. The program provides centralized intake, assessment, treatment case management, individual and group counseling, life skills follow-up and aftercare services. The participants are referred to the program from the criminal justice system, schools and other agency referrals. The program addresses the nuances of each adolescent's experience including cognitive, emotional, physical, social and moral development. There are also individual and group sessions for our monolingual Spanish speaking clients. The ICS/SSG YOUTH Program provides the youth in treatment with comprehensive wrap around services while remaining in their own community to improve treatment and service outcomes. The YOUTH aftercare program includes sober activities. Participants go on field trips such as attending professional sporting events, outdoor activities such as horse back riding, attending theatrical plays and participating in conferences to address issues concerning youth. The aftercare program staff believes that exposing and involving youth in positive, alternative social activities, will increase their self esteem and become better students in school and better young adults in life. | HOP/SSG Milagro Program | ||
5715 S. Broadway, Los Angeles , CA 90037 . The Milagro Program is a culturally competent outpatient treatment program for the Hispanic/Latino community that we serve in SPA 6. The program utilizes the Cognitive Behavior Therapy (CBT) model that has been modified to be more culturally appropriate for our Hispanic/Latino participants. The Milagro Program's use of the CBT treatment task is essential to successful substance abuse treatment. The tasks are 1) Fostering Motivation for Abstinence 2) Teaching Coping Skills 3) Changing Reinforcement Contingencies 4) Fostering Management of Painful Feelings and 5) Improving Interpersonal Functioning and Increasing Social Support . We offer a bilingual and bicultural alcohol and drug (AOD) treatment program for participants referred to us through Proposition 36. The Milagro Program addresses the issues of substance abuse, Alcohol and Drug Awareness Education, Individual and Family Development, Counseling, Self-Help Groups, Recovery Discussion, Relapse Prevention and HIV/AIDS. The program motivates and reinforces a drug free family perspective now and for in the future. The staff for the program is Alex Quiroz and Ernesto Quiroz. |

| Department of Mental Health Early and Periodic Screening, Diagnostic Treatment (EPSDT) |

5715 S. Broadway, Los Angeles , CA 90037 . DMH/EPSDT program is a component of our Youth program. Clients are referred to us through probation, other agencies as well as walk-in (self referrals). A therapist conducts an assessment to determine if the client has mental health and meets the criteria for services. If there is a mental health problem as the primary condition, the client is then enrolled for counseling services and a treatment plan is established. These services include individual, family and group counseling and case management. Each client is seen three (3) hours per week or more depending upon their treatment plan. These services are culturally sensitive and include bilingual staff. All services provided by or under the supervision of a licensed therapists. The staff includes Joan Lowe, Trina Williams, Lori Williams and Miguel Herrera. |

| Urgent Care Center |

Located at Augustus F Hawkins Medical Facility, Los Angeles, CA, the UCC provides services 24/7. Clients are able to receive services for up to 23 hours from the time they are admitted. The services provided include:
An integral part of these non-hospital based Urgent Care Services will be linking and following up with clients to ensure that they have the community supports needed, including Assertive Community Treatment programs, substance abuse treatment and housing. Linkage to ongoing outpatient services will be with DMH's directly operated and contracted providers, alcohol and drug programs that serve dually diagnosed clients, and specialized employment and housing providers. |

| Department of Mental Health AB2034 |

333 S. Central Ave. , Los Angeles , CA 90013 . AB2034 provides culturally and linguistically appropriate therapeutic intervention, crisis intervention, case management, and medication support including prescribing, administering, dispensing and monitoring of psychiatric medications necessary to alleviate the symptoms of mental illness, to transient adults who suffer from 1) chronic and persistent mental illness, 2) serious emotional disturbance, and 3) a co-existing disorder of substance abuse and homelessness. Clients are referred to us through our street outreach team, the county jail and walk-ins. A therapist conducts an assessment to determine if the client has mental health and/or substance abuse problems. The staff consists of mental health, substance abuse, outreach and case management professionals who work together to deliver these services designed to assist the client in gaining the social and functional skills necessary for appropriate development and social interaction. Program goals also include reducing jail recidivism and hospital admissions. The staff includes Cleante Stain, Keisha Owens, Robert Munoz and LaShawn Smith. |
